Clinicopathological Characteristics of Adult IgA Nephropathy in the United States

IntroductionIgA nephropathy (IgAN) is a progressive autoimmune kidney disease and leading cause of glomerular that can result in failure (KF). Median age at diagnosis 35-37 years close to 50% patients will progress KF within 20 years. We aimed enhance the understanding renal histology chronic (CKD) stage time IgAN using large real-world biopsy cohort.MethodsThis retrospective cohort study evaluated data clinical characteristics from adult US diagnosed with between 1 January 2016 31 May 2020. Descriptive statistics were summarized relationship(s) each Oxford Classification (MEST-C) component score 24-H proteinuria or CKD examined regression analysis.ResultsA total 4 375 (mean 47.7 years, 62.7% male) met eligibility criteria. Mild moderate mesangial hypercellularity (47.3%), segmental sclerosis (65.0%), tubular atrophy ≥ 25% (57.4%) crescents (18.5%) identified 74.6% 3. Proteinuria g/d was associated higher MEST-C scores, odds hypercelluarity, sclerosis, increased stage.ConclusionMost our ≥3 had high scores suggestive significant burden biopsy. Strategies are required raise awareness promote earlier detection asymptomatic urinary abnormalities before extensive irreversible damage has occurred.
